• Your name and library card number
• Purpose of meeting
• Name of organization (if applicable)
• Number of people expected
• Preferred location
• Potential date(s) and time(s) (meeting rooms are only available during library hours)
• Any extra requests – whiteboard, smartboard, computer access, etc.
